Consumers do not create accounts on My Benefit Pilot at this time.

2. Relationship to Agent Force (Affiliated Services)

My Benefit Pilot, Inc. and Agent Force, Inc. are separate legal entities that provide distinct services that may be integrated. When a consumer submits a contact form to an agent, that inquiry is routed to the agent and may appear in the agent’s Agent Force dashboard for the agent’s follow-up and recordkeeping.

My Benefit Pilot may share information with Agent Force, Inc. to operate integrated services.

3. Information We Collect

A. Information you provide through contact forms

If you submit a contact form to an agent through My Benefit Pilot, we collect the information you enter, which may include:

  • name
  • email address
  • phone number
  • message content
  • zip code
  • product interest

We also record the date and time the form was submitted and may record delivery status.

Important: Please do not include sensitive medical information or other sensitive personal information in the message field. We do not request it.
